General Requirements

1. Duly Accomplished Enrollment Form (Available at the registrar’s office or downloadable from our website)
2. Birth Certificate (Photocopy, PSA/NSO Certified)
3. Recent 2x2 ID Pictures (4 copies)
4. Medical Clearance or Health Certificate from a licensed physician
5. Good Moral Certificate from the previous school
6. Valid ID of Parent/Guardian (Photocopy)

For New Students (Freshmen & Transferees):

1. Report Card (Form 138) from the last school attended
2. Certificate of Transfer or Honorable Dismissal (for transferees)
3. Assessment or Placement Test Results (if applicable)
4. Special Education Needs Assessment (for students requiring accommodations)

For Returning Students:

1. Previous School Year’s Report Card (Form 138)
2. Updated Student Information Sheet
3. Clearance from Finance Office

Additional Requirements for Scholarship Applicants:

1. Scholarship Application Form (Available at the admissions office)
2. Income Tax Return (ITR) or Certificate of Indigency
3. Recommendation Letter from a community leader or previous school principal
